Schedule 1 – Particulars to be notified to the Executive or other notifying authority
- Date of notice.
- Principal contractor contact details.
- Address of the site or suitable description.
- Start date for construction phase.
- Name of local authority covering the site.
- Time allowed for planning and preparation.
- Description of the project and work.
- Planned duration of the construction phase.
- Client contact details.
- Estimated maximum number of people working on site.
- Principal designer contact details.
- Planned number of contractors on site.
- Name and address of any contractors already appointed.
- Name and address of any designers already engaged.
- Client declaration that they are aware of duties.

Schedule 2 – Minimum welfare facilities required for construction sites
- Sanitary conveniences should be sufficient in number, as clean as reasonably practicable, secure, adequately ventilated and lit.
- Washing facilities, including showers if required, to be provided so far is reasonably practicable at readily accessible places.
- Drinking water should be made available with suitable drinking method.
- Changing rooms and lockers should be provided as required.
- Facilities for rest including appropriate seating, should be provided at readily accessible places. These should also include the means for preparing food and boiling water.

- Risks of burial from earth falls, engulfment in swampland or falling from height.
- Risks of chemical and biological substances.
- Work with ionising radiation.
- Work near high voltage power lines.
- Risks of drowning.
- Work on wells, underground earthworks and tunnels.
- Work involving diving.
- Work within a caisson with a compressed air atmosphere.
- Work with explosives.
